Brief Description:
An incompatibility is preventing Two-Step Login from working properly in Chrome if LastPass is present and activated. LastPass is working on a fix.

Workaround:
In Chrome, when beginning a login that will result in Two-Step Login being triggered, use an Incognito window. Disabling the LastPass plugin or using a browser other than Chrome will also serve as a workaround.
Current Status:
LastPass has provided a new version of their Chrome plugin, which fixes the issue. There have been reports of Chrome autoupdating without requiring any action on the part of the user.
